When submitting a name change, include a certified copy of marriage license, divorce decree or other legal document supporting the name change.

The division of Enrollment Management (Admissions, Financial Aid, Registrar's Office) is committed to providing accurate, timely, courteous and thorough service to potential students, enrolled students, alumni and the university’s external constituents. The goal of each office within the division is to respond to written requests within three working days and telephone requests within 48 hours.
