CHECK-OUT PROCEDURE
Before leaving employment, employees are responsible for the completion of an Employee Check-Out Form that may be obtained from the department head or from Human Resources. The department shall complete the top portion of the form. The employee must then contact the various clearance points (preferably to make an appointment in advance of the actual visit) and, by personal appearance, clear all offices indicated on the form. In cases of termination, abandonment of position or if the employee is unable to return to the University, it is the department head/designee responsibility to call the areas for clearance. The completed form must be returned to Human Resources.
