FINANCIAL AID FORMS & APPLICATIONS

The following forms and applications are not available to be transmitted on-line but may be printed for mailing/faxing to our office.

  • Independent Verification Worksheet ~ 2006-2007

This form is used by independent students who have been chosen for verification by the federal processor when their Free Application for Federal Student Aid (FAFSA) was processed.

(Do not submit this form unless requested by the Financial Aid Office).

  • Dependent Verification Worksheet ~ 2006-2007

This form is used by dependent students who have been chosen for verification by the federal processor when their Free Application for Federal Student Aid (FAFSA) was processed.

(Do not submit this form unless requested by the Financial Aid Office).

This form is used when either the student, parent, or both did not, and were not required to file taxes. If this form was requested you must list all monthly living expenses and the source of funds by which they were paid.

(Do not submit this form unless requested by the Financial Aid Office).

  • Verification of Marital Separation ~ 2006-2007

If you indicated “separated” as your marital status on the FAFSA, you must provide documentation verifying that you are separated for the purpose of divorce, and that you are not receiving support from your spouse (other than child support or alimony).

(Do not submit this form unless requested by the Financial Aid Office).

  • New!!! Satisfactory Academic Progress Re-Evaluation Form ~ 2006-2007

Students who did not meet the satisfactory academic progress requirement of their Financial Aid or Scholarship may use this form to request a re-evaluation.

These forms are used by students who do not meet the definition of an independent student as outlined in the Free Application for Federal Student Aid (FAFSA) and would like to apply for a dependency change due to their unusual circumstance.

This form can be used to document a significant decline in the financial situation of the student, the student's spouse, and/or the student's parent(s). Changes may include a loss of job or benefits, extreme medical expenses, etc.. This request and all supporting documentation must be received by our office prior to the acceptance and disbursement of awards.

  • Concurrent Enrollment - UTT is my Home school ~ 2006-2007

Concurrent enrollment is a way to receive the Federal Pell Grant based upon the combined enrollment at UT Tyler and a local community college.

  • Financial Aid Cancellation Notice ~ 2006-2007

This form is used to cancel your financial aid award for the academic semester you indicate.  It will NOT withdraw you from classes or be sent to any other office on campus.

  • Request for additional Student Loans based upon a PLUS Denial ~ 2006-2007

This form may be used to request additional student loans based upon lender denial of a parent PLUS loan.

  • Transfer Request for Texas Grant or Be-On-Time Loan ~ 2006-2007

    This form is use to request the transfer of the Texas Grant or the Be-On-Time loan from another institution.

Through the IRS website, you may request a transcript of a previously submitted tax return or request proof that you were not required to file.

EXEMPTION & WAIVER FORMS

Texas needs more qualified teachers. That's why the State created a program that covers tuition and some fees for Education Aides who enroll in college to become certified Texas teachers.

To provide an education benefit to honorably discharged or separated Texas veterans.

  • Exemption Program for Children of Professional Nurse Faculty and Staff

  • Exemption Program for Clinical Preceptors and Their Children

~ Information & Application

To provide a partial exemption from the payment of tuition to eligible persons employed as clinical preceptors and to their children in order to encourage the preceptors to continue their employment and induce others to seek such employment in the State of Texas. SCHOLARSHIP FORMS

The University of Texas at Tyler Scholarship for Dependent Children of Employees is available for the children of full-time UT Tyler employees and children of retirees who meet the admission criteria and want to attend classes at UT Tyler on a full-time basis.

The University of Texas at Tyler Staff Development Scholarship is available to all full-time employees.

This form serves as a request for UT Tyler to apply monies to my account from The Texas Guaranteed Tuition Plan for the semester indicated below.  All students must be registered for classes prior to submitting the request.  This request is for ONE semester only.
