The following forms and applications (in PDF format) are not available to be transmitted on-line, but may be printed for mailing or faxing to our office.

Verification Forms

Independent Verification Worksheet ~ 2011-2012, 2012-2013
This form is used by independent students who have been chosen for verification by the federal processor when their Free Application for Federal Student Aid (FAFSA) was processed.
(Do not submit this form unless requested by the Financial Aid Office).

Dependent Verification Worksheet ~ 2011-2012, 2012-2013
This form is used by dependent students who have been chosen for verification by the federal processor when their Free Application for Federal Student Aid (FAFSA) was processed.

Parent Explanation of Low Income or Non-Filer Status ~ 2011-2012, 2012-2013
This form is used when the parent(s) did not, and was not required to file taxes. If this form was requested you must list all monthly living expenses and the source of funds by which they were paid.

Student Explanation of Low Income or Non-Filer Status ~ 2011-2012, 2012-2013
This form is used when the student did not, and was not required to file taxes. If this form was requested you must list all monthly living expenses and the source of funds by which they were paid.
(Do not submit this form unless requested by the Financial Aid Office).

Verification of Marital Separation ~ 2011-2012, 2012-2013
If you indicated “separated” as your marital status on the FAFSA, you must provide documentation verifying that you are separated for the purpose of divorce, and that you are not receiving support from your spouse (other than child support or alimony).
(Do not submit this form unless requested by the Financial Aid Office).

Qualification as Independent Student ~ 2011-2012, 2012-2013
Use this form, along with the acceptable supporting documentation, to prove your independent status.
(Do not submit this form unless requested by the Financial Aid Office). 

Statement of Selective Service Registration Status Form, 2012-2013
All students receiving an Exemption or Waiver at the University of Texas at Tyler, who have not filed the FAFSA  must complete the Statement of Selective Service Registration Status form.

 

Special Request Forms

Texas Grant Statement of Eligibility ~ 2011-2012, 2012-2013
This form is for all Texas Grant recipients who have received an Initial Award or a Renewal Award and must be signed and returned to the Financial Aid Office in order for the Texas Grant funds to be applied to student's accounts.

Financial Aid Re-Evaluation Request~ 2011-2012, 2012-2013
This form is used for requesting a re-evaluation of a student's financial aid based on childcare expenses, full-time enrollment or a change in a student's housing status.

Financial Aid Reinstatement Request ~ 2011-2012, 2012-2013
This form is used to reinstate financial aid awards that have been previously declined.

Financial Aid Cancellation Request ~ 2011-2012, 2012-2013
This form is used to cancel your financial aid award for the academic semester you indicate.  It will NOT withdraw you from classes or be sent to any other office on campus.

Satisfactory Academic Progress Re-Evaluation Request, 2012-2013
Students who did not meet the satisfactory academic progress requirement of their Financial Aid or Scholarship may use this form to request a re-evaluation.

Application for Appeal
Use this form to request an appeal for Financial Aid, Scholarships, or any other appealable circumstance in the Financial Aid Office.   Appeal requests MUST include this completed form and the appropriate supporting documentation.

Dependency Override Request ~  2012-2013
These forms are used by students who do not meet the definition of an independent student as outlined in the Free Application for Federal Student Aid (FAFSA) and would like to apply for a dependency change due to their unusual circumstance.

Special Circumstance Request ~ 2012-2013

This form can be used to document a significant decline in the financial situation of the student, the student's spouse, and/or the student's parent(s). Changes may include a loss of job or benefits, extreme medical expenses, etc.. This request and all supporting documentation must be received by our office prior to the acceptance and disbursement of awards.

 

Parent Plus Loan Forms

Federal Direct Parent PLUS Loan Application ~ 2011-2012, 2012-2013
This application must be completed by the Parent Borrower each academic year.

Direct PLUS Loan Authorization Form ~ 2011-2012, 2012-2013
Parent authorization for remaining PLUS Loan funds to be disbursed.

Request for Student Loans based upon a PLUS Denial ~ 2011-2012, 2012-2013
This form may be used to request additional student loans based upon lender denial of a parent PLUS loan.

Summer PLUS Loan Request Form ~ 2011-2012
This form is used to request the Parent PLUS Loan for the summer terms.


Forms For Students Transferring To or From UT Tyler 

Transfer Request for Texas Grant or Be-On-Time Loan ~ 2011-2012, 2012-2013
This form is use to request the transfer of the Texas Grant or the Be-On-Time loan from another institution.

Dual Enrollment Forms

Concurrent Enrollment - UTT is my Home school  ~ 2011-2012, 2012-2013
Concurrent enrollment is a way to receive the Federal Pell Grant based upon the combined enrollment at UT Tyler and a local community college. Colleges approved for Joint Baccalaureate Studies (JBS) at UT Tyler include Kilgore College, Panola College in Carthage, Trinity Valley Community College in Athens, Tyler Junior College, Jacksonville College, Lon Morris College in Jacksonville, Navarro College in Corsicana, Paris Junior College, and Northeast Texas Community College in Mt. Pleasant, and the Dallas County Community College District.
