UT Tyler Scholarship Forms
Letter of Appreciation - Beginning Fall 2012, scholarship funds will not be disbursed until the Letter of Appreciation has been received by the Enrollment Services Center. This document is required from every UT Tyler scholarship recipient.
UT Tyler Scholarship for Dependent Children of Employees
Faculty & Staff Development Scholarship Application- Students who completely withdraw from classes prior to the census date each semester will be responsible for any balances on their student account. The census dates for each semester can be found on the financial aid calendar.
UT Tyler Scholarship for Spouses of Full-Time Employees - Students who completely withdraw from classes prior to the census date each semester will be responsible for any balances on their student account. The census dates for each semester can be found on the financial aid calendar.
UT Tyler Employee Family Scholarship at TJC - This scholarship is managed by the Office of Human Resources at Tyler Junior College. For additional information, please contact the TJC Office of Human Resources at 903.510.2419.
National Merit Scholarship Request Form - Students receiving the National Merit Finalist Scholarship or the National Merit Semi-Finalist Scholarship must complete a request form each semester they plan to attend.
