CONFINED SPACE

 

7. EQUIPMENT AND MATERIALS

The following equipment and materials will be made available to employees and used when appropriate:

            Located in EH&S Department. Physical Plant:

  • Air monitor; MS 40 Multi-Gas Monitor, Industrial Scientific.
 

Provided as necessary;

  • Appropriate Fire Extinguisher
  • 2-way communication device (Walki-Talkie)
  • Appropriate eye protection.
  • Lockout devices.

 

8. EMPLOYEE TRAINING

Unless otherwise specified, employees entering confined spaces (entrants), confined space entry supervisors, and attendants must have received all of the training outlined below. Training shall be provided to each affected employee prior to assigned duties involving confined space operation [see 29 CPR 1910.146 (g) Training.].

  • Duties of entrants, attendants, and entry supervisors
  • Physical & Chemical hazards (H2S, Flammables, etc.)
  • Air Monitoring
  • Operation of Communication Equipment and Procedure

Note: All UT TYLER campus police are certified in First aid and CPR

 

9. RECORDKEEPING AND LABELING

  • A confined space entry permit (Appendix A) will be completed, signed and dated by a member of the EH&S Department or an individual designated by the EH&S Director. The original confined space entry permit shall be kept on file in the EH&S department for a minimum of three years. A copy of the entry permit will be forwarded to any department which required a permit.
  • Prior to entry, each confined space shall be identified by location and potential known hazards that may be associated with the space. Permit required confined spaces, when identified, shall be appropriately labeled "Danger - Permit Required Confined Space ­Authorized Entrants Only".
  • EH&S Department will maintain records of all confined spaces. Information to include location, description of area, anticipated hazards, etc.
