The forms below are for use by student organizations of The University of Texas at Tyler.  By selecting a form or form packet you will open a file that can be printed out and returned to the Office of Student Development in UC 111. In order for a student organization's registration to be considered complete, the representatives listed on the form must attend a Student Organization Orientation.
