Information for Faculty & Staff Traveling with Students

Staff and faculty traveling internationally with students on non-course related trips have the additional requirement of completing the International Travel Proposal, which is available upon request, and a Faculty or Staff-Led (whichever applies) Student International Travel Request.

OIP will also assist faculty and staff with collecting the required documentation from student travelers in accordance with Chapter 13 of the UT Tyler Manual of Policies and Procedures (MOPP).

Required Forms

UT Tyler faculty and staff planning to travel internationally must submit certain forms for travel approval prior to the expected travel. Below is a list of links to all individual faculty/staff international travel forms that must be submitted through DocuSign.

The International Travel Exemption Request is required only for travel to or through a country currently designated as a Level 3 (Reconsider Travel) Travel Advisory by the US Department of State.

A separate form will be provided by an International Travel advisor for faculty and staff to submit copies of their passport at the appropriate time.
