Please read and follow all instructions on each form carefully, provide all requested information and obtain any required signatures prior to submitting the form to the Enrollment Services Center (ADM 230). Incomplete forms will not be processed, and the student will be responsible for completing the form once contacted.

Note: For the purpose of signatures on the forms listed below, college advisors are defined as full-time or part-time employees whose primary role is academic advising of students. This includes advisors employed by a college, an academic department, or by the Office of Academic Success.

  • Faculty advisors are not eligible signatories for any document requiring a college advisor signature. Any document signed by a faculty advisor will be referred back to a college advisor.
  • Department chairs, assistant/associate deans and deans may sign in place of a college advisor from their college/department.

Students must present a valid government-issued photo ID or their UT Tyler student ID when turning in any form listed below. Some forms require additional documentation.
