Hazlewood Continuation-Required Once Every School Year

Complete each school year Fall-Summer. (Must have used Hazlewood at UT Tyler the previous school year or you must submit an initial application)

Submit only after June first and Spring Grades Have Been Posted.  Fall Continuations will not be accepted before then.

Complete at least 3 weeks prior to bill due date.  Students who submit their applications and/or certification requests less than 3 weeks prior to the bill due date will have to be prepared to pay their bill upfront, or take an emergency loan, or get on a payment plan if eligible.

Scan to PDF and EMAIL the following documents from your (link) UT Tyler Student Email Account to vrc@uttyler.edu.  

 

1.  Hazlewood Continuation Form

Hazlewood is applied for one school year.  Ie Fall-Summer (can not carry over multiple SCHOOL years)

 When filling out your continuation form put Fall-Summer in term TERM and 2017-2018 for the YEAR

 

AND

 

2. GPA Verification. Submit only after June first and Spring Grades Have Been Posted.  Fall Continuations will not be accepted before then.

Print your GPA from your MY uttyler and email a PDF copy with your application-Must show Spring Semester Grades and GPA for all fall continuations. If there has been a break  must show last semester attended here.

 

AND

 

3.  Hazlewood Hours  Log in and email us the print out(s) of hours

  (See below for the appropriate category and instructions) (Veteran and Student must register and create accounts even if they never used Hazlewood)

  • VETERAN you must log in and print off your VETERAN hours.  Will say- "Record of Standard and Legacy Hours Used".
    • (Not the Veteran's Student Hours)

 

  • LEGACY CHILD- (Veteran and Student must register and create accounts even if they never used Hazlewood before)
    • VETERAN must log in and print off their VETERAN hours.  Will say- "Record of Standard and Legacy Hours Used".

      (Not the Veteran's Student Hours)

                   AND

    • LEGACY CHILD must log in and print off their STUDENT hours. 

 

  • CHILD or SPOUSE of a TOTALLY and PERMANENTLY/100% disabled veteran (according to the VA) Log in and print your STUDENT hours.
    • (we do not need the veteran's hours in this case)

  

All documents must be Scanned to PDF and submitted together in one email from your UT Tyler student email account. Incomplete/partial applications are not accepted.

 

Check your UT Tyler Student Center and If there is still a bill the day before bills are due for the individual session, contact us then and we will address the issue. This is real important if you are using partial/less than 100% Chapter 33/Post 9/11 as it has to be manually calculated.

 

UT Tyler
