National Merit Scholarship Request Form - Students receiving the National Merit Finalist Scholarship or the National Merit Semi-Finalist Scholarship must complete a request form each semester they plan to attend.
Third Party Statement of Responsibility - All students utilizing a third party to pay all or part of their tuition and/or fees, must complete the Third Party Statement of Responsibility each semester.
Scholarship Satisfactory Academic Progress Re-Evaluation Request - Students who did not meet the satisfactory academic progress requirement of their Scholarship may use this form to request a re-evaluation.

Third Party Payment Programs
Texas Tomorrow Fund Request - Formerly known as the Texas Tomorrow Fund, it is a fund administered by the Texas Prepaid Higher Education Tuition Board that gives individuals the chance to prepay tomorrow's college tuition and required fees at Texas public and private colleges and universities at about what it would cost today. The Board invests these payments and will use the investment earnings to cover the full cost of college tuition and required fees at Texas public colleges and universities, or the estimated average private tuition and required fees at Texas private colleges and universities, for students enrolled in the program. An Account ID is required on the attached form before the application can be processed. If you did not have a family member who prepaid into a State fund, you do not qualify for the Texas Tomorrow Fund Request.
Texas Tuition Promise Fund - The Texas Tuition Promise Fund is designed to help families and individuals prepay for all or some future tuition and schoolwide required fees at most two- or four-year Texas public college or university (excluding medical and dental schools, health science centers and other health-related institutions). Account holders purchase Tuition Units, which represent a fixed amount of undergraduate resident tuition and schoolwide required fees charged by Texas public colleges and universities (excluding medical and dental schools, health science centers and other health-related institutions). The number of units needed varies depending on the school, but generally 100 units represents 30 semester hours, which is considered to be one academic year. An Account ID is required on the attached form before the application can be processed. If you did not have a family member who prepaid into a State fund, you do not qualify for the Texas Tomorrow Fund Request.
