How to Apply
Book an Appointment
- Note: Walk-ins will not be accepted
- Note: Appointments are only available to book two weeks in advance
- Can't find an appointment? Search for a nearby acceptance facility.
Complete the DS-11 Application Form
Collect All Required Documents
- Proof of U.S. citizenship
- Proper identification
- Passport photo (available onsite for $15)
- Check or money order for U.S. Department of State application fees (see below)
- Credit card for UT Tyler facility fees (see below)
- Determine any special circumstances

UT Tyler Passport Office
The Office of International Programs serves as a Passport Acceptance Facility for the UT Tyler community and the general public. OIP is authorized by the U.S. Department of State to execute and mail-in Form DS-11 passport applications. Passport and immigration-style photos are available for purchase separately, providing a convenient service to anyone filing a government application, including international students applying for practical training authorizations.
We are open to the public and happily serve our students, faculty and staff, East Texas community, state, and surrounding-state citizens.
