How to Apply

Book an Appointment

• Note: Walk-ins will not be accepted
• Note: Appointments are only available to book two weeks in advance
• Can't find an appointment? Search for a nearby acceptance facility.

Complete the DS-11 Application Form

Collect All Required Documents

• Proof of U.S. citizenship
• Proper identification
• Passport photo (available onsite for $15)
• Check or money order for U.S. Department of State application fees (see below)
• Credit card for UT Tyler facility fees (see below)
• Determine any special circumstances
