Biography
Hannah Holbert, PharmD, MPH, BCPS earned her Doctor of Pharmacy degree from the University
of Tennessee Health Science Center College of Pharmacy and her Master of Public Health
from the University of Memphis. She completed a PGY1/PGY2 Pharmacotherapy Residency
at the University of Tennessee Medical Center and the University of Tennessee Health
Science Center College of Pharmacy in her hometown of Knoxville, Tennessee. Prior
to joining FCOP, Dr. Holbert served as an inpatient clinical pharmacist at the University
of Tennessee Medical Center. She has developed extensive experience in both inpatient
and ambulatory care settings while contributing to didactic and experiential pharmacy
education.
Dr. Holbert is a Board Certified Pharmacotherapy Specialist (BCPS) whose primary clinical
interests include cardiology, transitions of care, and pharmacotherapy optimization.
Her professional interests also include pharmacy education, interprofessional collaboration,
and advancing patient-centered care.
