Previous State Service Verification forms are normally requested when transferring or gaining employment with another State institution in Texas.

Previous State Service Verification request are completed and returned within 3-5 business days of receipt of the request.

Instructions:

Via Email

• Please email to hr.leaves@uttyler.edu. At a minimum, requests must include current name, name when you worked at UT Tyler, last 4 digits of SSN.

Via Fax

• Please fax to 903-565-5690. At a minimum, requests must include current name, name when you worked at UT Tyler, last 4 digits of SSN.
