By selecting a form or form packet, you will open a file that can be emailed to getconnected@uttyler.edu or printed out and returned to Student Engagement in UC 3400. If you have any questions about any of the forms below, please view the Student Organization Handbook or call 903.565.5645.

Departmental Student Organizations

Groups that are going to be sponsored or partnering with departments will need to complete an agreement through Docusign naming them as "Department Sponsored Student Organizations" . If you think your group falls within this category or you would like to begin this process, please contact getconnected@uttyler.edu
